Pre-assessment Form

This pre-assessment form helps us understand your child's current development level, and consider the areas where early intervention support may be helpful. Once we have received your completed pre-assessment form, you will be on our waiting list for an initial assessment with our team.

Our initial Assessment is a play-based engagement where we explore the range of the child’s development and the aim of the parent in seeking our service. A written Assessment Report is provided to the parent/s within approximately 15 working days, outlining our observations, a summary of the information provided by the parent/s, child and parent/s goals, and a recommendation in regard to service.
